Maybe you want to turn your head and ignore it, but, everyone KNOWS that overhead is rising at an alarming rate for us dentists lately, but I'm not sure we realize just how much. I know you're thinking, "It's really not all that bad yet," but...

While most everyone around in 2005 had a thriving practice and were looking for ways to improve their already healthy bottom lines with more cosmetic dentistry and big case dentistry,
I was silently ashamed of my own practice's financial health.
Why was I failing?
I felt like I was on an island. I had all the same cosmetic and full mouth training as some of my friends and even the gurus who were all doing so well, still, my practice was floundering.
My practice in 2005 was set up to sell big cases and politely show patients asking for "regular" and emergency dentistry the door if they weren't ready to commit to doing all their dentistry now or rebuild their entire dentition to ideal condition.
I was proud of my skills. The sentence used around my office a lot was, "I'm sorry, but I can only recommend the same dentistry for you that I would put in my own mother's mouth."
My staff was trained to recommend only the "best." We blocked out large chunks of the schedule for big cases and stubbornly refused to let my scheduler fill in the time until the very last minute.
I was trying to be committed to success not give in to doing less expensive options.
I wanted that Taj Mahal, cover of the monthly dental digest, boutique practice badly.
It wasn't working, but I was proud and didn't want to give up. Still, somehow I knew deep down in my heart what kind of practice would work in my area (which was hitting an early recession due to factory closings.)
I would hear the phone ring and I would hear my well trained staff say, "I'm sorry, we don't accept your insurance." Or, "We'll be glad to get you in for an exam with the doctor. He likes to spend
at least an hour with each new patient and develop a plan just for you. I think we have a slot
next week." I cringe now remembering.
Do you see why I was hurting? I had been all over the country and taken all the wrong
advice. Sure, I had good clinical skills, but, in an economy where there is basically
NO DEMAND for those services, you will still sink like a rock. If you haven't planned properly for the possibility of an economy where people JUST WON'T BUY or spend money on anything that is not essential, you run the risk of financial disaster.
How Did I Turn It Around?
It wouldn't come easy, though. To go to the extreme other end of the dental spectrum from boutique dentistry would require extreme change on my part and the part of my staff.
The entire practice would have to be broken down and rebuilt piece by piece if we were going to be able to
pull this off in time to avert financial disaster.

First, I would have to win the hearts and minds of the staff. I thought it shouldn't be that hard because they could all see that the practice wasn't doing all that well. They also were experiencing a daily dose of rejection from all the patients brave enough to call the office and wanting to be seen soon.
No amount of scripts in the world could get the patients in my area to say YES to our offerings.
It must have seemed so foreign and expensive to the prospective patients who listened to us on the other end of their phone line.

They had been brainwashed and bought into the same crap that I had. I had to fix that first.

Step One Get my staff's mind right again. Apologize for leading them down the wrong path, and
paint the vision in their minds of what the future could hold if we changed the path our practice had been traveling. The beauty was that when things were all said and done, all we had to do was change our mindset, then execute a very simple organizational structure. We didn't even have to reinvent the model.
It has been successful for decades and exists in virtually all of the best companies and practices in all types of businesses in the world.
Step Two Get control of my Overhead. Where did I start? The #1 place to start was staff overhead. I know what you're thinking, right? You think I probably fired several people.
WRONG. There is a time and place for that nasty business, but you would be surprised how much you can make your staff payroll percentage go down by installing a very simple tool. them. After completing this last section, you won't be a CPA, but you will be able to at least ask the intelligent questions to your accountant.
